FAI court process begins for Addiewell prison death

The Crown Office has lodged a first notice to begin the court process for a fatal accident inquiry (FAI) into the death of 38-year-old Stewart Glancy.
Mr Glancy was a prisoner within HMP Addiewell who died on 20 June 2020.
A preliminary hearing will be held on 28 November 2025 at Livingston Sheriff Court. The FAI will be held from 2 to 6 February 2026 at Edinburgh Sheriff Court
The inquiry will explore the circumstances of Mr Glancy’s death, with particular focus on the prison’s welfare observations, record keeping and information sharing between NHS and SPS.
It will also consider whether the first aid training provided to staff was sufficient to allow them to properly respond and if the system in place at the prison was sufficient to prevent the distribution of non-prescribed drugs between prisoners.
